Information from the abstract

Abstract Background Kidney dysfunction is associated with adverse outcomes in cirrhosis, but the prognostic significance of longitudinal kidney function decline in compensated cirrhosis remains unclear. This study evaluated the association between kidney function decline and long-term clinical outcomes in compensated cirrhosis. Methods This retrospective cohort study included patients with compensated cirrhosis between 2016 and 2025. Kidney function decline was estimated using baseline and last available creatinine measurements during follow-up. The primary outcome was all-cause mortality. Secondary outcomes included sepsis-related hospitalization and hepatic encephalopathy. Multivariable Cox regression analyses were performed to evaluate associations between kidney function decline and clinical outcomes. Results Among 329 patients, 71 (22%) demonstrated rapid kidney function decline during follow-up. Rapid kidney function decline was independently associated with higher all-cause mortality (adjusted hazard ratio [HR] 9.25, 95% confidence interval [CI] 4.62–18.51; p < 0.001) and sepsis-related hospitalization (adjusted HR 3.65, 95% CI 1.75–7.62; p = 0.001). Diabetes mellitus, ascites, furosemide use, and prior hospitalization were more common among patients with rapid kidney function decline. Conclusion Rapid kidney function decline was independently associated with adverse long-term outcomes in patients with compensated cirrhosis. Longitudinal monitoring of kidney function using routinely available creatinine-based eGFR may help identify high-risk patients in the early stage of disease.
