Integrated technology acceptance model and theory of planned behavior explain anti-osteoporosis medication prescribing among Thai orthopedic surgeons after hip fracture

Information from the abstract

A persistent care gap leaves many patients without anti-osteoporosis medications (AOMs) after hip fracture, despite their proven benefit in preventing secondary fractures. We investigated the factors associated with orthopedic surgeons’ intention to prescribe AOMs. A cross-sectional online survey of 264 Thai orthopedic surgeons involved in hip fracture care integrated the technology acceptance model and the theory of planned behavior. Partial least squares structural equation modeling assessed the measurement model’s reliability and validity and tested the hypothesized structural relationships. The integrated model accounted for 63.4% of the variance in prescribing intention. Perceived usefulness of AOMs ( β = 0.427; P < 0.001) and subjective norms ( β = 0.228; P < 0.001) predicted the surgeons’ attitude toward use. Attitude toward use was the strongest predictor of the intention to prescribe AOMs ( β = 0.622; P < 0.001). Self-efficacy did not moderate the attitude–intention relationship but was directly associated with intention ( β = 0.201; P = 0.004). These results indicate that prescribing intention is primarily predicted by perceived clinical value and professional norms rather than by procedural simplicity. Interventions to reduce the care gap should therefore emphasize clinical effectiveness, reinforce professional norms, and build surgeons’ self-efficacy through targeted education and training.
