MXenes belong to a structurally diverse family of two-dimensional transition metal carbides, nitrides, and carbonitrides defined by the general formula M n +1 X n T x . Over the last decade they emerged as a functionally rich class of nanomaterials for use in diverse fields including energy, medicine, and electrochemistry. This review covers their synthesis from hydrofluoric acid (HF) etching of MAX phase precursors to safer alternatives including in situ HF-generating (LiF-HCl), molten fluoride salt, electrochemical, and hydrothermal/solvothermal routes, critically examining how each method determines surface termination spectra (–F, –OH, –O), structural defect density, colloidal stability, and biomedical translation scalability. Our main focus is the systematic comparison of HF versus HF-free synthesis from a biomedical perspective, indicating that etching chemistry directly governs toxicity of the material, functionalization capacity, and clinical suitability. The high electrical conductivity, tunable surface terminations, large specific surface area, and strong NIR photothermal conversion of MXenes support applications in photothermal, photodynamic, and sonodynamic cancer therapy; targeted drug delivery; multimodal imaging (MRI, CT, photoacoustic, fluorescence); electrochemical biosensing; tissue engineering; and antibacterial systems. We present a structured toxicological profile that covers oxidative stress, immunotoxicity, genotoxicity, hemocompatibility, and biodegradability, while clearly addressing the dose-dependent contradictions found in the existing safety literature. Surface functionalization approaches using PEG, PVP, Fe₃O₄, MnOx, and mesoporous silica are evaluated for their effects on biocompatibility and targeting efficiency. In addition to the well-studied Ti₃C₂Tₓ, we systematically review other non-titanium MXenes such as Nb₂C, Ta₄C₃, V₂C, Ti₂N, and Mo₂C across both therapeutic and diagnostic applications. A unique emphasis is placed on the potential of MXenes in treating neglected cutaneous parasitic infections, specifically cutaneous leishmaniasis and hookworm-related cutaneous larva migrans. We also identify and discuss key translational challenges including limited oxidative stability, inconsistent surface terminations, batch-to-batch variability, and the absence of GMP-compatible synthesis methods and show how our combined findings provide a practical approach for advancing MXenes toward precise diagnostics and targeted therapeutics.
