Aging reinforced concrete (RC) bridges are susceptible to complex deterioration, yet conventional visual inspections yield qualitative condition ratings that cannot be directly translated into remaining service life predictions. Meanwhile, standard theoretical fatigue models often overestimate damage accumulation when site-specific loading and material degradation histories are not accounted for. To bridge this gap between physical inspection and numerical simulation, this study proposes an integrated assessment framework that synergizes Unmanned Aerial Vehicle (UAV) photogrammetry with nonlinear Finite Element Analysis (FEA). Applied to an 80-year-old RC bridge in Thailand exhibiting extensive cracking, the methodology features a novel two-stage calibrated fatigue assessment. In the calibration phase, an uncertainty-based matching algorithm systematically compares UAV-derived crack topologies (including flexural crack spacing and shear crack inclination) against simulated failure patterns across the full FEA loading history to determine the structure's equivalent fatigue age and current damage index. In the prediction phase, a structure-specific S-N curve is derived from multi-level cyclic FEA simulations and combined with Miner's rule to project the remaining service life under variable traffic scenarios. The calibration revealed that the bridge has consumed approximately 83.3% of its fatigue life, with the uncalibrated model overestimating the damage rate by predicting the same damage state at 54 years versus the actual 80-year service life. Scenario analyses estimate a remaining service life of approximately 7.5 years under baseline traffic conditions, while a load restriction policy prohibiting heavy trailers is estimated to extend operational life by approximately 0.8 years, within a range of 0.4–1.3 years reflecting the confidence interval on the fitted S-N exponent. The sign of this benefit is robust across that range; its magnitude is not tightly determined. While the calibration inputs, namely crack pattern, load history assumptions, and material properties, are specific to the case-study bridge, the underlying two-stage procedure, comprising uncertainty-based crack-pattern matching followed by structure-specific S-N characterization and Miner's-rule projection, is a generic pipeline applicable to any RC bridge for which UAV crack data and a nonlinear finite element model can be obtained, including structures with incomplete historical load records. The proposed framework thus demonstrates a transferable methodology for converting field-observed crack data into quantitative fatigue life predictions for aging RC structures.
