Introduction Digital health applications and devices are increasingly used to support community-based elderly care. However, evidence on caregivers’ adaptation to these technologies and their impact on perceived health outcomes remains limited. This study aimed to examine caregivers’ adaptation to digital health technologies, their usage behaviors, and factors associated with perceived health impact in community-based elderly care. Methods An observational cross-sectional study was conducted among 160 caregivers working in Kantharawichai District, Maha Sarakham Province, Thailand. Data were collected using a structured questionnaire assessing knowledge, behavioral use, adaptation, satisfaction, and perceived health impact. Descriptive statistics, chi-square analysis and Fisher's exact test were applied. Results Caregivers frequently used health applications and digital health devices for monitoring elderly health and communicating with healthcare professionals. Overall satisfaction with digital health technologies was high, particularly regarding positive experiences with new technologies and organizational technological support (mean = 3.94). Knowledge of reliable digital health information sources was high (92.5%), although awareness of medication reminder applications was limited (31.9%). Most caregivers demonstrated high adaptation to digital health technologies (63.7%). Sociodemographic characteristics were not significantly associated with perceived health impact ( p > 0.05), except for average family income ( χ 2 = 5.903, p = 0.015; OR = 1.108, 95% CI: 1.040–1.180). Technology-related factors showed strong associations with health impact, including satisfaction with digital health technologies ( χ 2 = 35.346, p < 0.001) and behavioral use ( χ 2 = 4.437, p = 0.035). Conclusion Caregivers demonstrated high adaptation and satisfaction with digital health technologies. Strengthening caregivers’ digital competencies and supporting effective technology use may enhance community-based elderly care outcomes.
