Information from the abstract
Pharmacogenomic (PGx) biomarkers are essential for personalizing drug therapy, improving effectiveness, and reducing adverse reactions. This retrospective study aims to evaluate the detection of PGx biomarkers at a Thai Pharmacogenomic and Personalized Medicine (PPM) center at Ramathibodi Hospital, Bangkok, Thailand, between 2011 and 2024. In total, 40660 genotyped were evaluated successfully based on 25 genes compromising 5 human leucocyte antigens (HLA):-A, -B, -C,-DQA1, -DQB1 and -DRB1 along with CYP1A2, CYP2A6, CYP2B6, CYP2C9, CYP2C19, CYP2D6, CYP3A4, CYP3A5, DYPD, NAT2, ApoE, RYR1, CACNA1S, ITPA, MTHFR, NUDT15, SLCO1B1, TPMT, UGT1A1 and VKORC1. The highest test results were HLA-B (31285), followed by CYP2C19 (2797), TPMT (2364), and NUDT15 (662). The number of pre-emptive tests increased under Thai policies for prescribing drugs across years, including allopurinol, carbamazepine, and abacavir that indicated 15.3%, 14.9%, and 4.4% were carriers of HLA-B*58:01, HLA-B*15:02, and HLA-B*57:01, respectively. The high volume of TPMT tests (for Thiopurine drugs under policies) suggests precise dosing of anticancer medicines for at least 108 patients. Overall, detecting PGx biomarkers in a local center not only provides information to optimize drug dosage and prevent ADRs but also advances value-based health care, informs policymakers' decisions, and supports precision-guided therapeutic strategies.
